Missionary Questionnaire

Dear Missionary/Evangelist.

     Park Avenue Baptist Church would like to take a moment to thank you for planning to come to visit us. We have developed a short questionnaire. Please take a moment to fill it out so that we can make informed decisions on your stay. 

     Park Avenue Baptist Church looks forward to serving you. We are here for any needs that you may have. We would like to provide you with any accommodations or meals that you may need during your visit with us.
